In blender, purée milk, cottage cheese, cornstarch, Parmesan and garlic until smooth. Season lightly with salt and generously with pepper.
Transfer mixture to skillet over medium-low heat; simmer, whisking often, until sauce is bubbling and slightly thickened, 4 to 5 minutes. Serve over fettuccine and sprinkle with Parmesan and pepper, if desired.
